This section is designed for you to express your view of your health along with any topics not covered by the form that our Concierge Care can address for you.

• After filling out the form please save it for your records
• Print it out
• Do one of the following:
  • Mail the form to luxury@primaryhospital.com
  • Bring this form to your appointment.

• Establish if the free consult is in person or over the phone
• Sign a medical record release form
• Determine if the practice is right for you
• Determine long and short term goals for your future health
• Initiate a plan to achieve your new goals
